Vitamin D Will Not Fix Your Testosterone

Men who carry more vitamin D in their blood do tend to carry more testosterone, which is why the supplement pitch writes itself. Hand men vitamin D at random and wait, and the testosterone does not follow.

Randomised trials of vitamin D supplementation have not produced a clear rise in testosterone in men, and the trials run in men who were short of vitamin D found the same nothing.

The Short Answer

Vitamin D supplementation has not been shown to raise testosterone in men. The most recent pooled analysis of the randomised trials found no clear effect on total testosterone, or on any marker of how much of it is usable, and rated its own certainty low. The exception everybody reaches for, that it works if you are actually deficient, has now been tested three separate ways and did not hold in any of them. Get your level checked anyway, for reasons that have nothing to do with testosterone.

The Association Is Real and the Causal Claim Is Not

Correlation is where this argument starts, and it is also where it ends. The way you find out whether the vitamin is doing the work is to give it to half the men and a placebo to the other half.

That has now been done enough times to pool. A 2026 systematic review and meta-analysis in Nutrients, registered with PROSPERO before it ran and conducted to PRISMA 2020 and Cochrane methodological guidance, gathered 11 randomised comparisons covering 703 men. Total testosterone came out 0.47 nmol/L higher on vitamin D, with a confidence interval running from -0.50 to 1.44. An interval that straddles zero like that is the trials failing to agree on whether the effect is up, down or absent. SHBG landed at 0.27 nmol/L with an interval from -2.14 to 2.68. The free androgen index landed at -0.37. The review team graded their own certainty for total testosterone as low, and they were the ones who did the work.

The individual trials underneath that pool are worth reading on their own, because between them they cover the three situations a man reading this can actually be in.

Start with the easy one. Healthy men, testosterone already normal at a mean of 19.1 nmol/L, vitamin D below target at a mean of 53.3 nmol/L. Twelve weeks at 20,000 IU a week moved total testosterone by a median of 0.5 nmol/L, p = 0.497. That trial ran in the Journal of Clinical Endocrinology and Metabolism in 2017, and the version of record sits behind a paywall, so what is reported here is what its published abstract states.

Correcting a Genuine Deficiency Did Not Move Testosterone Either

This is the good objection. Of course nothing happened in men who already had enough. Fix a real shortage and the hormone should follow.

It has been tested three ways.

The subgroup table. A 2024 meta-analytic review in Diseases is the only pooled analysis that splits the trials by whether the men were vitamin D deficient at baseline. Neither half reached significance. The deficient men came in at 0.16, across 8 studies, with an interval of -0.00 to 0.33. The men who were not deficient came in at 0.57, across 7 studies, with an interval from -0.71 to 1.85. The larger point estimate belongs to the group with nothing to correct. Those figures are weighted mean differences, and the paper never states their unit anywhere in its text, tables or figure captions, so they are printed here exactly as it prints them.

The trial built to catch it. 100 middle aged men, mean age 47, recruited for having low testosterone and low vitamin D at the same time. Total testosterone below 10.4 nmol/L on entry, so every man in it had room to move. Twelve weeks at 20,000 IU a week, 94 finishing, testosterone measured by mass spectrometry rather than immunoassay. No treatment effect on the primary outcome, and none on free testosterone, the free androgen index, SHBG, estradiol, FSH, LH, body composition or metabolic markers. That trial and the 2017 one both came out of the same group in Graz, sharing a first author and five co-authors, so treat them as two shots from one lab rather than two labs agreeing.

Actual repletion, for six months. 228 vitamin D deficient young men, mean baseline 25(OH)D of 21.5 nmol/L, randomised in a two by two design with calcium. Cholecalciferol at 60,000 IU a week for eight weeks and then fortnightly, until the supplemented groups were above 75 nmol/L, which is the part that matters: the deficiency was genuinely corrected rather than under dosed. Six months later the change in serum testosterone was similar in all four groups, and the same held in the men who had started below 25 and below 12 nmol/L. That study is paywalled too, so it is reported here from its published abstract, and testosterone was a secondary outcome sitting behind handgrip strength. Mean age 20, which is not a 42 year old in Dubai, though it is the age group most likely to show a hormonal response if there is one to show.

Nobody has yet run the trial in men with confirmed deficiency and diagnosed low testosterone together, with testosterone as the prespecified primary outcome. Until that exists, the deficiency case is unproven rather than disproven.

The Positive Result Shrinks as the Trials Get Better

There is a pooled analysis that found a rise, and it deserves naming rather than burying. That same 2024 review in Diseases gathered 17 randomised trials in 1,774 men and reported a small statistically significant increase in total testosterone overall, at 0.38 with an interval of 0.06 to 0.70 across the 15 studies in that model. Again, no unit. Three things sit beside it.

Free testosterone, the fraction that actually does anything, came out at 0.00.

The only two subgroups that reached significance, trials running longer than 12 weeks and trials dosing above 4,000 IU a day, have smaller point estimates than the subgroups that did not. 0.13 and 0.12, against 0.68 and 0.75. Duration and dose bought tighter confidence intervals, not a bigger effect. Nobody should read that table as take more, for longer.

And the 2026 review, which applied stricter rules about units and dispersion and only pooled the comparisons it could verify, did not reproduce the finding.

That is not one team being right and another being wrong. It is a literature getting fussier about what it will pool. The newer review is more recent and more selective, not larger. On total testosterone it pools 703 men against 1,774, and it is more selective precisely because it threw out what it could not check.

One last objection, because you will meet it on the first page of results. There is a 2011 trial that most supplement pages lead with. The 2026 review assessed that exact comparison with Cochrane's risk of bias tool and rated it one of only two high risk of bias comparisons in the whole evidence base, and the same review names structured weight loss as one of the co-interventions that make estimates unreliable. The men in that trial were in a weight reduction programme, and losing weight raises testosterone on its own.

Get Your Level Checked for Your Bones, Not Your Testosterone

None of this says vitamin D does not matter. It has established roles in bone health and immune function, and those are outside the question this article is answering. What the evidence does not support is buying it as a testosterone lever.

So the blood test is still worth having. A 25(OH)D reading is cheap and routine, and knowing your number tells you something real about your bones. It just does not tell you anything about your testosterone, and no figure in this literature justifies a specific dose or a target level for that purpose, so this page will not give you one. Every trial above was run in men, and none of it says anything about women.

This week, if testosterone is the thing you are actually chasing, spend the attention where the evidence lives instead. Train hard enough to matter, eat enough to support it, and protect the hours you sleep. And before the next bottle goes in the basket, the list of supplements that survive this kind of scrutiny is three items long.
